1. I do hereby entrust Lazy Days Boarding, to care for my pet/s during its boarding stay and agree to the terms and conditions listed below.
2. I authorize Lazy Days Boarding to do whatever is deemed necessary for the health and well being of my pet/s and agree to reimburse Lazy days Boarding all expenses incurred.
3. I have provided my contact details for home and holiday location, where possible, and agree to be contacted by lazy Days Boarding with reference to my pet/s welfare. If for any reason contact cannot be made, I entrust and authorize Lazy Days to make financial decisions concerning my pet’s welfare as stated at para 2.
4. I declare that to the best of my knowledge my pet/s are free from infectious diseases and I have declared to Lazy Days Boarding any health or aggression problems my pet/s may have.
5. Lazy Days will not accept any pet that is showing signs of illness, disease, pregnancy or injury. We will not be held responsible for any of the afore mentioned conditions. We ensure all pets are fed a well balanced diet, and are kept as safe as possible, we will not be held responsible for the loss of or the contraction of any illness such as Myxomatosis, VHD, snuffles, diarrhoea, fly strike, mites or other common diseases identified during and after the boarding of you pet at Lazy Days Boarding.
6. I agree to allow my pet to be vaccinated in the event if a local outbreak of any contagious disease.
7. In the event of non-collection of pets, I agree to be contacted. Where upon I will be informed of a suitable collection time. I also agree to make payment for the extra days of boarding as a result of non-collection of my pet/s. If I fail to collect my pets for a period exceeding 7 days, without prior agreement, I understand that my pet will be put up for adoption and court action will be taken for reimbursement of boarding fees.
8. All costs incurred during non-collection will be charged at the normal rate and to the signatory of the Boarding Contract.
9. It is a known fact that many small animals can pass away with very little or no warning and very quickly after first showing signs of illness. This is very distressing for you the pet owner and us at Lazy Days. Every care and respect is given during these sad times with regards to you and your pet/s.
10. In the event of unfortunate circumstances I agree to be contacted, to enable myself to express my wishes concerning my pet/s burial, cremation or disposal. I also accept full responsibility for the removal of my pet/s from Lazy Days Boardings Premises, and agree to pay for any costs.
11. Lazy Days Boarding will accept no responsibility for any unfortunate circumstances or accidental loss of pets and will not be liable for any financial recompense concerning these situations.
Payment must be made in full before boarding commences, each part day will be charged at the full rate.
